The attachment of a foreign citizen to a polyclinic is carried out by:

Monday - Friday from 08.00 to 21.00

Saturday from 9.00 to 18.00

It is necessary to present originals and photocopies of the following documents:

Passport 1 and 3 pages
Insurance policy from 2 sides

Application forms are in the register.
